Cover page, Table of Contents, Abstract/ Executive Summary  

Learner submits a concise, organized summary of the document (abstract), a table of contents, and a cover page. Abstract and tables of contents. The abstract/ executive summary provides an overview of the entire document. Each component is one page or less.

 Needs Statement or Problem Statement

Learner submits organized, coherent , & logic al  needs statement or problem statement.  It includes the nature, extent, and impact of the problem, the factors contributing to the problem, and effective research-based theoretical approaches and practical  interventions that are closely related to the program you are proposing for funding.  It addresses accessibility, awareness, availability, appropriateness, and acculturation, if relevant.  The statement is supported by at least five reliable sources of literature. This section is 1-2 pages long.

Project /Program

Learner submits an organized, coherent, and concise project description. Learner describes the program. This includes: the programs’ background/ history; the programs’ the target population (including demographics, risk factors, and why there is funding needed for the population); the programs’ vision, goals and objectives (Including process, outcome, and impact objectives, as relevant) ; the programs’ implementation plan (a detailed description of the approaches, methods, and activities that need to be funded that directly relate to the goals and objectives), and the programs’ timeline and scope of delivery plan (a detailed plan of activities with corresponding schedule).  Staff needs are addressed; learner provides a detailed description of number of staff, their qualifications and job descriptions, and specific assignments. Chain of command is discussed. This section is 2-6 pages. Supporting literature is used where needed. Charts and graphs are included where needed.

Evaluation Plan 

Learner provides a clear concise evaluation plan.  Learner explains evaluation methods  including  assessment of overall impact and success of the program and explains data to be collected and its importance. Evaluation plan clearly aligns with the elements of the program /project section. This section is 1-2 pages.

Applicant Capability

Learner delineates agency commitment to proposal and ability to support it. This section is 1 page.

Budget 

Learner itemizes the expenditures and includes a rationale or justification for each expense. This section also includes a description of other funding sources and indicates the plan to continue the program beyond the funding period, if applicable. No page limit.

Appendix / Conclusion

Learner provides an appropriate  appendix with all necessary materials, as needed. This includes items such as job descriptions, data collection instruments, new forms, supporting letters, etc. An appropriate concluding paragraph is included.

Grammar, APA, References

Learner provides APA formatted reference list if applicable. Learner used correct APA, spelling, grammar, and punctuation. Learner stays within page limit.

Individual Participation

Learner submits a written statement about their participation in the group project. This includes  a list of the role(s) played and tasks completed .

Learner submits a written statement about editing of the paper; either peer or writing center.

Presentation:

Presentation

Learner  identifies the funding source, the problem/need statement, the program (i.e. background, goals, objectives, methods, and timeline), the evaluation plan, the budget, and any additional information as appropriate.  Learner identifies all the key point from their written proposal.  Learner describes key points to writing an effective grant proposal or discusses obstacles their team faced and how they overcame them. Learner’s demonstrates professional good practices as identified in the readings on development and presentation of  an effective presentation. Learner’s presentation is creative.
